What scabies is and how to recognise it

Scabies is an infestation of the skin by a microscopic mite that burrows into the outer layer to lay its eggs. Your immune system reacts to the mites and their waste, producing a furiously itchy rash that is typically worst at night. First-time infestations can take two to six weeks to cause symptoms, so you can pass scabies on before you know you have it; people who have had it before usually start itching within days.

Look for thin, wavy, slightly raised burrow lines and clusters of small red bumps in the classic locations: the webbing between the fingers, the wrists, elbows and armpits, around the waistline, and on the buttocks and genital area. Scabies spreads mainly through prolonged skin-to-skin contact with someone who is infested. It is common among couples and travellers sharing close quarters, and it has nothing to do with hygiene — anyone can catch it.

How we diagnose and treat scabies at the clinic

Walk in at any hour and a doctor will examine the rash under magnification, looking for burrows and the tell-tale distribution; where the picture is unclear, a gentle skin scraping can be checked for mites and eggs. Once scabies is confirmed, you receive a prescription scabicide cream from our in-house pharmacy with step-by-step instructions — applied to the whole body from the neck down, washed off after the recommended time, and usually repeated about a week later to catch newly hatched mites. In selected cases the doctor may recommend an oral medicine instead, and antihistamines or a soothing cream can calm the itch while your skin recovers.

One point matters more than any other: everyone who has had close skin contact with you in the past month should be treated at the same time, even if they feel fine. Expect some itching to continue for two to four weeks after successful treatment — this is your skin still reacting to dead mites, not a sign the cream failed. If you see new burrows or a clearly spreading rash several weeks on, come back and we will reassess.

Stopping reinfestation — and when to seek urgent care

On treatment day, machine-wash recently used clothing, towels and bed linen in hot water and dry them on a hot cycle; anything that cannot be washed can be sealed in a plastic bag for at least 72 hours, since the mite cannot survive long away from human skin. There is no need to fumigate your room or discard your luggage.

Scratching can break the skin and let bacteria in. Seek medical care promptly — day or night — if an area becomes increasingly red, warm, swollen or painful, weeps pus, develops red streaks, or if you run a fever: these are signs of a bacterial skin infection such as cellulitis.
