What shingles is and how to recognise it

Shingles is a reactivation of the varicella-zoster virus, the same virus that causes chickenpox. After chickenpox the virus lies dormant in the nerve roots and can wake up decades later, travelling along a single nerve to the skin. Age, stress, recent illness and lowered immunity make reactivation more likely — which is why long trips are a common backdrop to a first episode.

The illness often announces itself before anything is visible: burning, tingling or stabbing pain in one patch of skin. Within two to three days a rash of small fluid-filled blisters appears in a band on one side of the body — most often across the ribs or waist, sometimes on the neck, face or around one eye. That one-sided pattern separates shingles from an ordinary skin rash.

Although its medical name is herpes zoster, shingles is not the same infection as herpes simplex, the virus behind cold sores and genital herpes. The viruses are related but the treatment plans are not interchangeable — a good reason to let a doctor look at the rash rather than guessing.

How we treat shingles at the clinic

Diagnosis is usually clinical: the doctor examines the rash, asks when the pain and blisters began, and checks for warning signs such as eye involvement, so treatment normally starts at the first visit. If the rash is under 72 hours old, prescription antiviral tablets can begin straight away. Started early, antivirals shorten the illness, limit new blisters and reduce the risk of postherpetic neuralgia — the stubborn nerve pain that can outlast the rash by months. Even after three days it is still worth being examined, particularly for older patients or severe rashes.

Shingles can be genuinely painful, so the doctor will also build a pain-relief plan and show you how to keep the rash clean, dry and loosely covered while it heals. Blisters usually crust over within 7 to 10 days, and most people feel back to normal in two to four weeks. If you develop a fever, or the blisters become increasingly red, hot or weepy, come back — those can be signs of a secondary skin infection.

When shingles needs urgent attention

Most cases heal without complications, but some situations need a doctor urgently. Come in immediately, at any hour, if the rash involves your eye, eyelid or the tip of your nose, since the virus can threaten sight; if facial weakness, ear pain or hearing changes appear alongside a facial rash; if blisters spread widely beyond a single band; if your immune system is weakened by HIV, chemotherapy or transplant medication; or if you develop confusion, a severe headache or a stiff neck.

A note for travellers: you cannot give another person shingles, but blister fluid can pass the virus to anyone who has never had chickenpox or the chickenpox vaccine. Until every blister has crusted over, keep the rash covered, skip pools and massages, and keep your distance from pregnant women, babies and anyone with lowered immunity.
